Medical reports are patient information provided by medical officers/physician. The application is for the purpose of insurance claims, court trails, employer, police and etc. Without this report, claim processes or court trials cannot be carried out.

There are 3 (three) types of medical reports which are commonly requested by patients/patient’s heirs.

1. Standard Medical Report prepared by the Medical Officer
2. Concise Report / Opinion prepared by the Physician
3. Detailed Medical Report prepared by the Physician
