Request For Certificate or Diploma (Graduate Schools)

●Certificate applications for programs managed by the Academic Service Office for the Life and Environmental Sciences Area (as of April 7, 2025)

Due to restrictions on entry to some facilities from the 2025 academic year, the Academic Service Office for the Life and Environmental Sciences Area will suspend accepting certificate applications in person for the time being and will only accept applications by mail.\ We apologize for any inconvenience caused and appreciate your understanding and cooperation.

●Suspension of certificate issuance service in Fiscal Year 2025

\

The service of certificate issuance will be suspended as following schedule.\ The mail service also will be suspended during the period. Therefore, Issue of certificate(s) by mail may take longer than usual around the schedule.\ We are sorry for the inconvenience and thank you for your cooperation.

* Remarks

\

In case you request for a certificate other than the university specified form, please contact the respective office in advance.\ Certificates are issued at respective academic offices. Individuals enrolled in more than one course are requested to contact the respective office in charge when requesting more than one certificate.\ Transcript/Certificate of Completion and Transcript/Certificate of Enrollment written in English are not available. Transcript, Certificate of Enrollment and Certificate of Completion written in English will be issued separately.

How to Apply

You can either submit the form to the respective office or post the application form. When requesting by mail, be sure to enclose the completed application form and a self-addressed envelope.\ (Request by telephone or fax will not be accepted.)

* Note: For Certificates of Non-degree Research Students/MEXT Japanese Language Intensive Course Students, the application procedures is described the bottom of this page.

Academic Service Office on Tokyo Campus (Office for the Evening Programs for Working Adults)

Please note that the Academic Service Office on Tokyo Campus will only issue certificates for those who have completed the graduate school program at Tokyo Campus. For those who have completed the programs at Tsukuba Campus, please contact the respective office on Tsukuba Campus.

2. Request by Postal Mail

(1) Request for Certificates

Please request after filling out the requisition form. The form is downloadable from the link below.\ Request for Issuance of Certificates (Form for Graduate)\

(2) Self-addressed Envelope

• In case send to the applicant lives in Japan\ \ If you wish to receive the certificates by mail, you are asked to bear the postage charge incurred. Be sure to enclose a stamped self-addressed envelope. Please calculate the postage cost by yourself, referring to the website of the Japan Post.\ \ (For Reference)\ 1 certificate: 5 g\ 1 confidential envelope: 5 g \ \ * Please make sure to put enough postage stamps on an envelope.\ \ The following information is for your reference.\ A Certificate of Completion: 1 sheet of paper\ A Transcript: 1-2 sheets of paper\ \ * We would like you to ask to understand beforehand that we cannot respond to cases such as not including the reply envelope or an accident during the delivery. \
• Application by proxy\ \ A letter of proxy is needed to let a proxy receive your certifications or to send an envelope addressed to the proxy. The letter of proxy must be written in the applicant's original handwriting with his or her signature and seal. (Format: Proxy letter) When the proxy comes to receive a certificate at the Inquiry window, his or her identification card (like a driver's license card) is needed for ID purpose.\ \ Format: Letter of proxy （PDF 56KB）\ * If the "proxy letter" form above cannot be used, you can fill out necessary matters in any format.(Style Free) \
• Regarding applications from overseas\ \ If you have a proxy (family/friends) in Japan, we recommend that asking them for your application. Please refer above.\ If you do not have a proxy, the postage costs will be paid by the applicant. In such a case, send us one of the following items and the completed acquisition form.\ \ Make sure to include a return envelope that has written your name and address (Mailing address).\ A stamped return envelope (Japanese stamps)\ A return envelope and International Reply coupons\ \ * International Reply coupons:\ It is like a voucher common to each country and can be redeemed for the country's stamps on the receiving side.\ For details, please check the homepage of Japan Post and the post office of each country. \ \ If you will use the private courier company (Fedex, DHL, etc.),\ The invoice that has paid transportation charges is required and the envelope the company provided.\ Also, the University is not responsible for any loss or damage caused during the delivery.\
