We encourage those who seek to report an incident of concerning behavior to fill out the form below, which will allow us to direct the information to the appropriate campus authorities. If you are in immediate danger or require medical attention, please contact the police or emergency medical services at 911.\ \ To ensure your report gets to the attention of the correct team, please begin by first selecting your UCLA affiliation below. Behavior does not have to be illegal to be of concern, and any of the following reports may be submitted anonymously.
