Access your personal [Prehealth Portal](https://web.miniextensions.com/d8km1Aqewp7MQ9qe0tjt) by logging in with your N number and password. If you don't know what your password is, email tandonprehealth@nyu.edu. If you know your password but want to reset it, you can do so via your [Tandon Prehealth Profile](http://about:blank). If you cannot login, fill out the appropriate form below:
