The following is a list of links to forms you may need access to while enrolled as a graduate student at The University of Texas at Austin. If you don't find a form listed here, it is possible that policy requires you to submit a petition or letter with required signatures. Review the page where the form is hosted for more information.
Graduate Student Form Submission
Access the form
All of the following materials should be submitted to the Graduate School via the Graduate Student Form Submission:
- Request for Final Oral Examination
- Change in Committee Request
- Add/Drop Form: Late adds, delete drops and multiple add/drop requests only. For Q/F drops and grade status changes, please have students submit via the registrar’s Q-Drop & Grade Status Change system instead. (No Add/Drop Form required)
- Report of Dissertation Committee (Ph.D.)
- Master’s Committee Approval
- Request for Delay of Publication
- Transfer of Credit
- Withdrawal
- Leave of Absence
- Late Registration
